We try to make booking as easy and convenient as possible! Select the button below to schedule your appointment. The software (JaneApp) will ask you to make an account, which will allow you to keep tabs on all your appointments, reschedule or cancel as needed, and will ensure you get reminders prior for your appointments!

Please note, you can select OPT OUT for any marketing emails upon setting up your email and password through the Jane software. 

AT YOUR CONVENIENCE

After booking, you can edit, cancel or schedule new appointments through our website, just click Book Appointment to be redirected to the JaneApp software. It will again ask you to login. 

After booking, you will receive a confirmation of booking email as well as reminder emails or texts depending on your preference. 

INTAKE FORM

If you have never been here before, you will be sent an Intake form to complete prior to attending your appointment.

 

This form helps us to understand your therapeutic goals,  history and to check for any contraindications to Massage Therapy. You will also be able to input your insurance information for direct billing.

DIRECT BILLING

We direct bill for most insurance companies. Please check from the list below to confirm we are able to submit to your plan. 

Please note: We cannot guarantee you will be covered fully or at all. We do not know the specifics of each patient's plan and it is therefore your responsibility to contact your benefits company to confirm the amounts covered. We also cannot guarantee that the insurance portal will be performing maintenance, at which time we will not be able to bill for you. Please come prepared to pay for your appointment, should direct billing not be possible or what you expected.
